What Is a Deep
Plane Facelift?

If you have been researching facelifts, you have almost certainly encountered the term “deep plane.” It appears on surgeon websites, in social media posts, and across patient forums — often described as the gold standard of modern facial rejuvenation. But what does it actually mean, and how is it different from the facelifts you may have seen a generation ago? A deep plane facelift is a surgical technique that lifts and repositions the deeper layers of the face — the muscle and connective tissue that sit beneath the skin — rather than simply tightening the skin itself. This deeper approach produces results that look more natural, last significantly longer, and avoid the tight or “pulled” appearance that many people associate with older facelift techniques.

It Is Not Just Your Skin That Ages

When most people think about facial aging, they think about skin: wrinkles, fine lines, loss of elasticity. And while the skin does change over time, it is not the primary driver of the visible changes that lead people to consider a facelift.

The deeper issue is structural. Beneath the skin lies a layer called the SMAS — the Superficial Musculoaponeurotic System. This is a continuous sheet of muscle and connective tissue that envelops the deeper structures of the face. It supports the fat pads, defines the contour of the cheeks, and maintains the definition between the jawline and the neck.

As you age, the SMAS descends. The ligaments that anchor it to the underlying bone gradually weaken, and the entire layer shifts downward and forward. This is what creates jowling along the jawline, deepens the nasolabial folds (the lines from the nose to the corners of the mouth), hollows the midface, and blurs the once-clean angle between the jaw and the neck.

This is a structural change — and it is why treatments that address only the skin surface (creams, lasers, even surgical skin tightening) can only do so much. They are treating the symptom while the underlying cause continues to progress.

How a Deep Plane Facelift Works

In a deep plane facelift, the surgeon dissects beneath the SMAS layer, releasing it from the ligaments that tether it to the bone. This allows the SMAS — along with the fat pads, muscle, and overlying skin — to be lifted and repositioned as a single composite unit.

This is the critical distinction. The skin is not separated from the deeper tissue and pulled independently. Instead, everything moves together. Because the skin is never placed under tension, there is no risk of the tight, windswept look that older techniques could produce. The result is a face that looks restored rather than tightened — as though time has been gently reversed.

The deep plane approach also preserves the blood supply to the skin. In a traditional facelift, the skin is undermined (separated from the underlying tissue) over a wide area, which can compromise blood flow and increase the risk of complications. In a deep plane facelift, the dissection occurs beneath the SMAS, leaving the skin’s blood supply largely intact. This supports more predictable healing and reduces the risk of skin-related complications.

How Deep Plane Differs from a Traditional Facelift

In a traditional facelift — sometimes called an SMAS plication or SMASectomy facelift — the surgeon lifts the skin, then addresses the SMAS as a separate layer, either folding it (plication), trimming and suturing it (SMASectomy), or tightening it in place. The skin is then re-draped over the repositioned SMAS and the excess is removed.

This can produce visible improvement, but the results depend in part on tension applied to the skin. Over time, as the skin stretches, the result can relax and fade. Traditional facelifts typically last three to seven years.

A deep plane facelift eliminates this dependence on skin tension. Because the deeper structures and skin are moved as one unit, the result is maintained by the repositioned SMAS itself — a far more durable foundation. This is why deep plane results typically last 10 to 15 years.

The visual difference is also significant. Traditional facelifts can sometimes produce a “pulled” or “operated” appearance, especially around the ears and hairline, because the skin has been stretched to achieve the lift. Deep plane facelifts, because they do not rely on skin tension, produce a more natural, restored appearance.

How Deep Plane Differs from a Mini Facelift

A mini facelift — sometimes marketed as a “weekend facelift” or “short-scar facelift” — is a less invasive procedure designed for patients with mild to moderate signs of aging, particularly early jowling and lower face laxity. It typically involves a shorter incision, a more limited SMAS treatment, and a shorter recovery.

A mini facelift is not a lesser version of a deep plane facelift. It is a different procedure for a different stage of aging. For patients with mild concerns, it can deliver excellent results with less downtime. For patients with moderate to significant aging — deeper jowling, substantial midface descent, pronounced nasolabial folds — a mini facelift may not provide adequate correction, and a deep plane facelift is more appropriate.

The right choice depends entirely on your anatomy and the degree of change you are experiencing. This is why an in-person assessment with an experienced surgeon matters more than any article can convey.

What Results Does a Deep Plane Facelift Produce?

The goal of a deep plane facelift is not to create a different face. It is to restore the one you had — the structure, the contour, the definition that time has gradually shifted.

Patients who undergo a deep plane facelift typically describe the result as looking like themselves again: rested, refreshed, and naturally youthful. Others may notice the improvement but are unable to pinpoint what changed — which, for most patients, is exactly the goal.

Because the deeper structures have been repositioned rather than pulled, the result ages gracefully. You will continue to age after a deep plane facelift, but you will always look younger than you would have without it. Most patients describe it as turning back the clock by approximately a decade.

Results typically last 10 to 15 years, compared to three to seven years for a traditional facelift. This longevity is one of the primary reasons the deep plane technique has become the preferred approach among fellowship-trained facial plastic surgeons.

Is a Deep Plane Facelift Right for You?

A deep plane facelift is generally best suited for patients who are experiencing moderate to significant facial aging that non-surgical treatments — fillers, Botox, skin tightening devices — can no longer adequately address. Most patients are between 45 and 70, though candidacy is based on anatomy and health rather than age alone.

You might be a good candidate if you are noticing jowling along your jawline, deepening folds around the nose and mouth, loss of definition in the midface, or a general heaviness to your face that makes you look tired even when you are not.

It is not about reaching a specific age. It is about reaching the point where surface-level treatments are no longer delivering the results you want — and you are ready for a lasting, structural solution.

The Role of the Surgeon

Not every surgeon who offers a “deep plane facelift” performs the technique in the same way. The depth of dissection, the degree of ligament release, and the precision of repositioning vary based on the surgeon’s training, volume, and anatomical understanding.

When evaluating surgeons, the questions that matter most are: How many deep plane facelifts have you performed? Are you board-certified in plastic surgery by a recognized body (in Canada, the FRCSC; in the United States, the ABPS)? Do you have fellowship training? Can I see results on patients with anatomy similar to mine?
