Deep Plane Facelift Under
Local Anesthesia

For many patients considering a deep plane facelift, the procedure itself is not their biggest concern. It is the anesthesia. The idea of general anesthesia — being unconscious, intubated, and entirely dependent on a machine to breathe — carries an emotional weight that can be a genuine barrier to moving forward.

What most patients do not realize is that a deep plane facelift does not require general anesthesia. At Niagara Plastic Surgery Centre, Dr. N. S. Huq routinely performs deep plane facelifts under local anesthesia or TIVA (twilight sedation) — and he is one of the few surgeons in Canada who does so.

This is not an experiment. It is a refined approach backed by published research and decades of experience.

Dr. Huq is the Guest Editor of an Elsevier textbook on local anesthesia in plastic surgery — a comprehensive publication covering the science, technique, and clinical application of local anesthesia across the full range of plastic surgery procedures. His 2024 poster presentation, “Investigation of the Safety and Outcome of Abdominoplasty Without General Anesthesia: A Scoping Review,” was presented at the Canadian Society of Plastic Surgeons, McMaster Medical Student Research Day, and Niagara Health Knowledge Institute Research Day.

Local anesthesia for a deep plane facelift involves numbing the surgical area completely while you remain awake and alert. You feel no pain during the procedure — the area is fully anaesthetized — but you are conscious, breathing on your own, and able to communicate with Dr. Huq and his team throughout.

For patients who prefer a middle ground between full consciousness and general anesthesia, TIVA (Total Intravenous Anesthesia) — commonly known as twilight sedation — is also available. TIVA provides deep relaxation and drowsiness through intravenous medication while avoiding the full unconsciousness and intubation of general anesthesia. Many patients drift in and out of light sleep and have little memory of the procedure, but they are never on a ventilator.

The benefits of avoiding general anesthesia are real and well-documented:

No nausea or vomiting. Post-operative nausea is one of the most common and unpleasant side effects of general anesthesia. Under local anesthesia, this simply does not occur. Patients often eat a normal meal the same evening.

No grogginess or cognitive fog. General anesthesia leaves many patients feeling disoriented, groggy, and fatigued for 24 to 48 hours after surgery. Under local anesthesia, patients feel alert and clear-headed from the moment the procedure ends.

Faster initial recovery. Because the body has not been subjected to the systemic effects of general anesthesia, the first 24 to 48 hours of recovery are typically more comfortable. This head start compounds over the first week.

Reduced risk. General anesthesia carries inherent risks including airway complications, adverse drug reactions, and cardiovascular effects. Local anesthesia eliminates these risks entirely. For patients with health conditions that make general anesthesia higher risk, local anesthesia may make the procedure accessible where it otherwise would not be.

Lower cost. General anesthesia requires a physician anesthesiologist and additional monitoring equipment. Local anesthesia and TIVA generally cost less, which can contribute to a lower all-inclusive quote.

Patients who undergo a deep plane facelift under local anesthesia frequently describe the experience as far more comfortable than they expected. The numbing process takes a few minutes and involves a brief sensation of pressure. Once the area is fully anaesthetised, the procedure is painless.

During the surgery, you may feel gentle pressure or movement, but no pain. Many patients listen to music, talk with the surgical team, or simply rest quietly. The atmosphere is calm and unhurried.

Under TIVA, the experience is even more relaxed — most patients are in a comfortable, drowsy state and recall very little of the procedure while remaining physiologically stable without intubation.

After the procedure, you are able to walk out of the clinic on your own. There is no recovery room delay waiting for anesthesia to wear off. Most patients are home within an hour of the procedure ending.

For patients who strongly prefer general anesthesia — whether for personal comfort or medical reasons — it remains fully available at NPSC. All general anesthesia cases are supported by Canadian board-certified FRCPC physician anesthesiologists (not nurse anesthetists). The facility is fully equipped with emergency resuscitation equipment, including a crash cart, and Greater Niagara General Hospital is located half a block away.

The choice between local anesthesia, TIVA, and general anesthesia is made collaboratively between you and Dr. Huq during your consultation, based on your comfort, your health, and his assessment of what will produce the safest, best outcome.

The reason most deep plane facelifts are performed under general anesthesia is not that it is medically necessary — it is that most surgeons are trained to operate under general anesthesia and have not developed the expertise to perform complex procedures under local.

Performing a deep plane facelift under local anesthesia requires a surgeon who is exceptionally comfortable with the technique, who can manage the numbing precisely, and who can operate efficiently in a setting where the patient is awake and aware. It requires a different skill set and a different temperament.
